Radiation Licence

Step 1:

The HaBS Placement Ready Team will send you an email to prompt you to complete your application. A letter will also be emailed to you from UQ via InPlace, confirming you are a current student and have completed the necessary training to receive a Radiation Licence.

Step 2:

Download your Enrolment Status Report from mySi-Net as this will show evidence of your graduation date, indicating to the board that you require a 3 year radiation licence. If you have not included this in your application, you can email it to the licensing board. 

Step 3:

Fill out this Radiation Licence application form (PDF, 885.5 KB).

TIP: Save the form and fill it out digitally as there are drop-down menu selections in many of the boxes that you can select regarding student application. e.g to select the fee exemption choice in all the payment boxes.

Please ensure you do the following:

• In Part E on Page 2,  tick the Radiation Apparatus box and then the first two boxes as shown in the image below:

• You must provide two (2) certified copies of your ID with your application as detailed in Page 3 on the application form. This means that a Justice of the Peace/Commissioner of Declarations/Notary Public (or equivalent if you are currently overseas) must witness, sign and certify these (please ensure an official stamp/seal is applied). 
• The UQ Letter you receive from the HaBS Placement Ready Team AND your Enrolment Status Report which you will send with the application form will confirm with Queensland Health that you are a completing a Bachelor of Dental Science (Honours) or Doctor of Clinical Dentistry at UQ. You DO NOT need to provide copies of any further qualifications.
• International students: You may apply for Radiation Licence while in your home country. If you do not have a postal address in Australia, you may list the following address on your application:

You will need to go to the address above upon your arrival to Brisbane to collect your Radiation Licence.

Step 4:

Submit your completed application directly to Queensland Health via email - licensing@health.qld.gov.au OR as per the address on the application form. Remember if it is via email to ensure it is one PDF with all the evidence and the PDF file size is smaller than 10MB. 

Step 5:

Once you have sent your application to Queensland Health for processing, you will receive your licence in the mail to the address nominated in your application.

Please upload evidence of your licence to InPlace for the HaBS Placement Ready Team to verify.

Step 1:

The HaBS Placement Ready Team will send you email reminders 6 weeks before your Radiation Licence expires to prompt you to complete your application. They will also send you a letter from InPlace confirming you are a current student and have completed your relevant training to renew your Radiation Licence. *If you are asked to provide evidence of your graduation date, you can download and send evidence of your Enrolment Status Report from mySi-Net. 

You should receive a renewal form from Radiation Health QLD when your licence is due to expire. This will be sent to the postal address on your original application. If you have since moved, you will need to contact Radiation Health to update your contact details:

Step 2:

Sign the renewal document and submit your completed renewal form and the UQ Confirmation Letter directly to Queensland Health. You can post it or you can now renew online via Public Health Connect (PH Connect).

Step 3:

Once you receive confirmation of your renewed Radiation Licence, upload your evidence to InPlace for verification by the HaBS Placement Ready Team.
